If you are new to the Model of Care submission process we are also able to walk you through the submission process.
Care Management Operations
From comprehensive program gap analysis to care management remediation we are able to help improve care management process and efficiency, documentation, review of assessments and tools as well as documentation. Special attention is given to ensure you are following all of your internal policies and procedures, your model of care, and regulatory requirements.
Auditing
Desk review to full Mock Audit we are able to review your program against your Model of Care, CMS or NCQA requirements, and policies/procedures to identify any areas of risk your organization may be open to during a SNP Care Coordination Model of Care Audit. We are also able to provide assistance and support through a CMS and can be onsite or virtual depending on your needs.
Training
